This page is a compilation of forms you will need to send in with genetic tests, to become a member of Chessie or other retriever-related organizations, or to get more information on topics listed in this website. Links to forms are embedded within various articles on this site; they are listed again here to make it easier to find them. A note about filling out forms: Many times, I am unable to link up a dog's record with his or her OFA clearances, CERF number, etc. This is because there is some kind of inconsistency, missing information, etc. on the OFA or CERF form when you send it in. To avoid having your dog's record incomplete, please take care to review your dog's information on the form against his or her registration certificate. Differences in spelling of your dog's name, or incomplete/inaccurate registration numbers will result in the data not being all together. Whether you fill out the form, your vet does, or your vet's assistant, it is always a good idea to check it for accuracy before sending it in. Also, many dogs appear in the OFA or CERF databases as having no permanent identification. OFA has a form that allows you to correct this information if your dog has a tattoo or microchip.

 

Health Test Forms

Cardiac Form - for certification of clearance for evidence of genetic heart conditions. Your own vet can do this test, or you can have it done by a cardiologist. You will need to have this form with you when your vet listens to your dog's heart, as most vets do not have this form.

DM Test Order Form/Shopping Cart - order your dog's Degenerative Myelopathy test kits through this secure ordering portal.

EIC Test Form - print off and send in with samples. Instructions for doing cheek swabs are included on this form.

Elbow Form - send in with your dog's x-rays. Can be used for preliminary as well as permanent OFA numbers, and to re-do hips and/or elbows.

Hip Form - send in with your dog's x-rays. Can be used for preliminary as well as permanent OFA numbers, and to re-do hips and/or elbows.

OCD (Shoulder) Form - This is performed via x-ray, so can be done at the same time as hips and/or elbows. Most vets do not have this form, so bring with you if you wish to have this test done.

Patellas - can be checked at any time, without anaesthesia. Bring this form with you to your vet.

PRA: Online OptiGen Form - Order your PRA test online.

PRA: Printable OptiGen Form - Printable form; please be sure to include all the pages.

Sebaceous Adenitis - occurs in Standard Poodles and has been reported in Chesapeakes, though rarely.

Thyroid - Send to the testing laboratory along with your dog's blood sample.

 

Other Health Forms

DNA Test Form - If you had your dog's DNA tested for EIC, PRA, or MPS-VI, you will need to fill out this form and send it in to OFA to get an OFA number and have the information added to OFA's database.

ID Verification Form - your vet can scan your dog's microchip or visually verify your dog's tattoo and fill out this form. This will convert all your dog's OFA numbers to "VPI" or Verified Permanent ID status.

Open Database Form - For for changing your dog's status from Closed Database to Open Database. This is used only for dogs that do not pass one or more tests. Dogs who pass automatically show up in the database.

Chesapeake Bay Retriever Club of Canada (CBRCC) - Parent Club for the breed in Canada.

Canadian Kennel Club (CKC) - Membership application for Canadian residents.

Canadian Kennel Club (CKC) - Membership application for non-Canadian residents.

Finger Lakes Retriever Club - NY-based retriever club, holds monthly training days, offers seminars and holds two AKC Hunt Tests each year. All breeds welcome, Chessie-friendly hunt test club.
